Applying to Become a CLSA IPA Services Client
If you are interested in having CLSA serve as your IPA, you will need to complete the CLSA IPA Services Contact Form and submit it to CLSA electronically. Once CLSA receives your information, it will evaluate the situation and then contact you by email, SMS text, or telephone.
Completion and submission of the CLSA IPA Services Form is not a guarantee that CLSA will accept you as a client for IPA Services. CLSA must preliminarily feel a transfer is in your best interest for it to accept you as a client which it cannot do until it has evaluated your information. Even if you are accepted as a client, there is no guarantee a Court will approve any proposed transfer.

Please fill in the information on the CLSA IPA Services Contact Form. All of the information will be kept confidential. The information will help CLSA determine if you are a viable candidate for IPA Services with CLSA. If you do not know the answer to a specific question, write “I do not know at this time.” If you think you know but are not certain, write “I believe this is correct but am not certain.” Do not leave any entry blank. CLSA will rely on your responses in its evaluation.
